What does each individual need to achieve their personal best?
At the Cambian Group we know that progress and improvement can mean different things to different people. That's why our in-house multi-disciplinary team are drawing on the success achieved in Mental Health Rehabilitation services and are currently implementing a unique set of measurements called Active Care.
Using the Active Care system we will define and set step-by-step objectives across all areas of an individual's life. We will also provide the support, encouragement and environment needed to achieve them.
As well as encompassing psychological and behavioural objectives, Active Care embraces a wide range of specific activities including:
- Foundational daily living skills
- Personal hygiene
- Laundry
- Shopping
- Cooking
- Road safety
- Community-based skills
- Attending college
- Vocational activities
- Leisure pursuits
- Personal development skills
- Relationships and interpersonal skills
- Behaviour management
- Coping and tolerance skills
- Anger management
Every Active Care programme is unique. By setting targets to match each person's specific needs assessment, Active Care gives the multi-disciplinary team an accurate point of reference to help measure progress.
