The care pathway

Individualised care pathways

We're proud that many of the individuals who leave our care successfully take steps towards a more independent future. That's because our individualised care pathway are in line with the personalisation agenda and give each person the right mix of security, independence and basic skills education to match their specific needs.

Our pathway supports individuals at every stage of their particular journey from:

  • Low secure units for men and women
The focus here is on assessment and treatment

  • Locked intensive rehabilitation units for men and women
Support and security with increased independence and improved community access

  • Community step-down hospitals for men and women
Providing direct access to mainstream services and improved skills for daily living skills

  • Specialist residential homes
Longer term placements for those requiring some suport

  • Supported living
Because everyone is entitled to their "own front door"

  • Day opportunities
Providing positive and meaningful opportunities for adults
