Active Care™ Therapy Input
Cambian's unique programme combines intensive rehabilitation therapy with patient-centred care plans determined by individual needs.

Each patient is supported by a multi-disciplinary team (MDT) of psychiatrists, psychologists, occupational therapists, nurses, social workers and family members. This ensures that every area for development is covered, and provides the structure and the experiences needed to maximise potential.
Step-by-step objectives detailed on Cambian’s Active Care Wheel© allow progress in all areas of a patient’s life to be measured. A total of 6 wheels cover different phases of a patient’s stay with Cambian.
The information acts as a point of reference for the MDT.
Which stage has the patient reached in each quadrant? What should we do next? How does progress in one segment affect another? Improvements can be celebrated and consolidated. Areas of difficulty can be identified and new objectives set.
In a graphic representation, patients can see for themselves the progress they have made – the more segments completed, the nearer those segments are to the circumference of the wheel, the closer the patient is to achieving the skills necessary to leave Cambian services and rejoin the community.
Our hospitals contain state-of-the-art facilities where patients can practice and master tasks in a non-threatening environment. A high staff ratio ensures personal attention and one-to-one skill building.
Trips to local amenities, if appropriate for the individual, are built in to the programme to provide opportunities for further independence training.
Everything we do is with an eye to the future, planning for each patient’s permanent return to the community or suitable community-based care.
